(ii) the supervisor is no longer able or willing to continue to supervise Dr. MOIDEEN’s practice, or

(iii) three years from the date of issuance or when Dr. MOIDEEN has received certification from the Royal College of Physicians and Surgeons of Canada, whichever comes first.
