Undertaking by Dr. Gunter Helge Koch to the College of Physicians and Surgeons of Ontario (the “College”). (NOTE: This is a summary of the undertaking. For the complete terms, contact the College’s Registration Department):


(3) I, GUNTER HELGE KOCH, hereby resign from the College, having previously advised the College of my decision to retire from medical practice at age 65. (4) I undertake not to apply or re-apply for registration as a physician or for a licence to practise medicine in Ontario or any other jurisdiction. (5) I acknowledge that, in exchange for this undertaking, the College agrees to take no further action with respect to the Investigations. (6) I agree that in the event that the College should became aware that I have either applied, re-applied or attempted to apply or re-apply for registration as a physician or for a licence to practise medicine in any jurisdiction, the College shall have the right to proceed with a disciplinary proceeding on the basis of a breach of this undertaking and, if successful, shall have the right to re-commence the Investigations it terminated as a result of this undertaking, and/or proceed with a referral of specified allegations to the Discipline Committee. (8) I consent to the terms of this undertaking being entered on the register as information that is available to the public.
