On March 23, 2026, the Ontario Physicians and Surgeons Discipline Tribunal found that Dr. Iracleous committed the following acts of professional misconduct:
failing to respond appropriately or within a reasonable time to a written inquiry from the College; and
engaging in an act or omission relevant to the practice of medicine that, having regard to all the circumstances, would reasonably be regarded by registrants as disgraceful, dishonourable or unprofessional.
On the same date, the Tribunal ordered that:
the registrant attend before the panel to be reprimanded;
the Registrar revoke the registrant’s certificate of registration effective immediately; and
the registrant pay the College costs in the amount of $6,000 by April 22, 2026.

Dr. Iracleous will practise under the guidance of a Clinical Supervisor acceptable to the College for six months.
Dr. Iracleous will engage in professional education in medical recordkeeping, communication with patients, the indications for prescribing antibiotics, and formulating appropriate differential diagnoses and management plans for patients presenting with reduced oral intake, difficulty swallowing, and weight loss.
Dr. Iracleous’ practice will be reassessed by an assessor selected by the College within six months of the end of the period of Clinical Supervision and the completion of the professional education.
A summary of a decision of the Inquiries, Complaints and Reports Committee in which the disposition includes a "caution-in-person" is required by the College by-laws to be posted on the register, along with a note if the decision has been appealed. A “caution-in-person” disposition requires the physician to attend at the College and be verbally cautioned by a panel of the Committee. The summary will be removed from the register if the decision is overturned on appeal or review. Note that this requirement only applies to decisions arising out of a complaint dated on or after January 1, 2015, or if there was no complaint, the first appointment of investigators dated on or after January 1, 2015.
