  • prescribing medications for herself, including for controlled and/or monitored drugs, using the credentials of two colleagues without their knowledge or consent; and
  • failing to maintain appropriate boundaries in her interactions with Patient A.
The Tribunal ordered that: 
  1. Dr. Cook attend before the panel to be reprimanded; 
  2. the Registrar suspend Dr. Cook’s certificate of registration for six (6) months commencing January 28, 2025 at 12:01 a.m.; 
  3. the Registrar place terms, conditions and limitations on Dr. Cook’s certificate of registration; 
  4. Dr. Cook pay the College costs in the amount of $6,000 by February 3, 2025.
