As from 12:01a.m., August 29, 2025, by order of the Inquiries, Complaints and Reports Committee of the College of Physicians and Surgeons of Ontario, the following terms, conditions and limitations are imposed on the certificate of registration held by Dr. Hadi Shojaei:
(1) Practice Restrictions – Office Practice
(i) Subject to the exceptions set out in paragraph (2) below, Dr. Shojaei shall not engage in any professional encounters, in person or otherwise (“Professional Encounter” or “Professional Encounters”), with patients of any age, in any jurisdiction, unless the Professional Encounter takes place in the continuous presence and under the continuous observation of a monitor who is a regulated health professional acceptable to the College (the “Practice Monitor”), and unless the other requirements provided in this Order are fulfilled. For further clarity, Dr. Shojaei must not be alone with any patient, for any length of time, during any Professional Encounter, whether or not the parent or guardian of the patient is also present.
(ii) Dr. Shojaei shall review the Practice Monitor’s undertaking, attached hereto as Appendix “B” and understand what is required of the Practice Monitor including, but not limited to:
a. providing reports (as described in the Practice Monitor’s undertaking) to the College on at least a monthly basis;
b. remaining present at all times during all Professional Encounters, in person or otherwise, with all patients;
c. carefully observing all of Dr. Shojaei’s Professional Encounters with patients, including but not limited to physical and internal examinations, and to ensure the Practice Monitor’s view of all of Dr. Shojaei’s Professional Encounters with patients, including physical and internal examinations, is unobstructed at all times;
d. refraining from performing any other functions, except those required in the Practice Monitor’s undertaking attached as Appendix “B”, while observing Dr. Shojaei in all his Professional Encounters with patients;
e. maintaining a log of all Professional Encounters with patients in the form attached to this Order as Appendix “C” (the “Log”;
f. initialing all corresponding entries in the records of patients noted in the Log; and
g. submitting the original Log to the College on a monthly basis.
(iii) Dr. Shojaei shall maintain an up to date copy of the Log by ensuring a copy is made at the end of each business day, and to make it available to the College upon request.
(2) Practice Restrictions – Hospital Practice
(i) Dr. Shojaei shall not engage in any professional encounters, in person or otherwise (“Professional Encounter” or “Professional Encounters”), with patients of any age, in any jurisdiction, in his hospital practice, unless the Professional Encounter takes place in the continuous presence and under the continuous observation of a monitor who is a regulated health professional ( “RHP monitor” or “RHP monitors”), and unless the other requirements provided in this Order are fulfilled. For further clarity, Dr. Shojaei must not be alone with any patient, for any length of time, during any Professional Encounter in his hospital practice, whether or not the parent or guardian of the patient is also present.
(ii) Dr. Shojaei shall maintain a log of all Professional Encounters with patients in his hospital practice in the form attached to this Order as Appendix “C” (the “Log”), and ensure that any RHP monitor observing a Professional Encounter initials the Log. Dr. Shojaei shall make the Log available at any time to the College upon request.
(iii) Dr. Shojaei’s RHP monitors shall:
a. remain present at all times during all Professional Encounters with patients in Dr. Shojaei’s hospital practice;
b. carefully observe all of Dr. Shojaei’s Professional Encounters with patients, including but not limited to physical and internal examinations. Dr. Shojaei shall ensure the RHP monitors’ view of all of his Professional Encounters with patients, including physical and internal examinations, is unobstructed at all times;
c. initial the Log being maintained by Dr. Shojaei for any Professional Encounter observed by the RHP monitor; and
d. initial the corresponding entries in the records of patients observed by the RHP monitor.
(3) Dr. Shojaei shall inform the College of each and every location at which he practises, delegates or has privileges including, but not limited to any hospitals, clinics, offices, and any Out-of-Hospital Premises and Independent Health Facilities with which he is affiliated, in any jurisdiction (collectively his “Practice Location” or “Practice Locations”), within five (5) days of this Order. Going forward, Dr. Shojaei shall inform the College of any and all new Practice Locations within five (5) days of commencing practice at that location.
(4) Posting a Sign – Office Practice
(i) Dr. Shojaei shall post a sign in all waiting rooms, examination rooms and consulting rooms, in regards to all office Practice Locations, in a clearly visible and secure location, at all times whether or not he is physically present at the office Practice Location, in the form set out in Appendix “D” that states: “Dr. Shojaei must not have professional encounters, in person or otherwise, with patients of any age, unless in the continuous presence and under the continuous observation of a practice monitor acceptable to the College of Physicians and Surgeons of Ontario. Dr. Shojaei must not be alone during any professional encounter with any patient. Further information may be found on the College website at www.cpso.on.ca”.
(ii) For further clarity, Dr. Shojaei shall not be required to post a sign in the hospital setting.
(iii) Dr. Shojaei shall post a certified translation in any language in which he provides services, of the sign described in section (4)(i), in all waiting rooms, examination rooms and consulting rooms, in all of his office Practice Locations, in a clearly visible and secure location, in the form set out at Appendix “D”, as applicable.
(iv) Dr. Shojaei shall provide the certified translation described in section (4)(iii), to the College within thirty (30) days of this Order.
(v) Dr. Shojaei shall, if he elects, after the execution of this Order, to provide services in any other language, notify the College prior to providing any such services.
(vi) Dr. Shojaei shall provide to the College the certified translation described in section (4)(iii) prior to providing services in any language described in section (4)(v).
(5) Notifying Patients
(i) Dr. Shojaei shall ensure that each patient with whom he has a Professional Encounter is directly notified, prior to the Professional Encounter, of the details of the restrictions described in section (1)(i) above, as applicable.
(ii) With respect to patients with whom he has appointments that are scheduled at least seven (7) days in advance, Dr. Shojaei, shall ensure that each patient is directly notified, within seven (7) days after the appointment is scheduled, of the details of the restrictions described in section (1)(i), above, as applicable.
(iii) For further clarity, Dr. Shojaei shall not be required to notify patients in the hospital setting.
(6) The College may accept an alternative arrangement with respect to practice monitoring, signage and patient notification in the hospital setting, in its sole discretion.
(7) Dr. Shojaei shall be solely responsible for payment of all fees, costs, charges, expenses, etc. arising from the implementation of any of the provisions of this Order.
(8) Dr. Shojaei shall provide this Order to any chief of staff, or a colleague with similar responsibilities, at any Practice Location (“Chief of Staff” or “Chiefs of Staff”).
(9) Dr. Shojaei shall consent to the College making appropriate enquiries of OHIP and/or any person who or institution that may have relevant information, in order for the College to monitor his compliance with the provisions of this Order.
(10) Dr. Shojaei shall submit to and not interfere with, unannounced inspections of his Practice Locations and patient records by the College and to any other activity the College deems necessary in order to monitor his compliance with the provisions of this Order.
(11) Dr. Shojaei shall consent to the College providing the following information to all Practice Monitors and RHP monitors carrying out duties under this Order:
a. any information the College has that led to this Order;
b. any information arising from any investigation into, or assessment of, Dr. Shojaei’s practice; and
c. any information arising from the monitoring of Dr. Shojaei’s compliance with this Order.
(12) Dr. Shojaei shall consent to all Practice Monitors and RHP monitors carrying out duties under this Order to disclose to the College, and to one another, any of the following:
a. any information relevant to this Order;
b. any information relevant to the provisions of the Practice Monitor’s undertaking set out at Appendix “B”;
c. any information relevant for the purposes of monitoring Dr. Shojaei’s compliance with this Order.
(13) Dr. Shojaei shall consent to the College providing all Chiefs of Staff with any information the College has that led to this Order and/or any information arising from the monitoring of Dr. Shojaei’s compliance with this Order.
(14) Dr. Shojaei shall consent to all Chiefs of Staff disclosing to the College, and to one another, any information relevant to this Order and/or relevant for the purposes of monitoring Dr. Shojaei’s compliance this Order.

1. Dr. HADI SHOJAEI may practise medicine only,
(i) in a setting that is approved by the Chair, Department of Internal Medicine, Division of Clinical Sciences, Northern Ontario School of Medicine, in which Dr. SHOJAEI holds an academic appointment at the rank of ASSOCIATE/FULL Professor, and
(ii) in accordance with the requirements of their academic appointment.
2. The certificate of registration automatically expires when Dr. SHOJAEI no longer holds the appointment, referred to in clause 1.
This certificate is issued on July 1, 2025, and currently has an interim expiry date of June 30, 2026. The expiry date of this certificate may be extended, subject to the limitations in paragraph 2 above.
